I authorize Prime Achievement Records to use materials I submit, where lawfully permitted, in PAR publications, productions, websites, licensed/promotional materials and other media for recognition, archival and communication purposes. I understand submitted materials may not be returned. I understand that the safety of participants is my/our responsibility. I will take appropriate precautions, check relevant equipment and comply with applicable health, safety, venue and legal requirements. PAR recognition does not replace required insurance, permits, professional supervision or regulatory compliance. I declare that the information and supporting material provided by me is true and accurate to the best of my knowledge.
